Cardiac Care Measures
In collaboration with the American Heart Association, ÃÛÌÒÖ±²¥ established standardized measures for Comprehensive Cardiac Center (CCC) Certification, which was launched in 2017. In the spring of 2018 a Technical Advisory Panel (TAP) met to review proposed existing cardiac performance measurement requirements for certified CCC organizations.
The following are Comprehensive Cardiac Care (CCC) chart abstracted measures used by ÃÛÌÒÖ±²¥.
Comprehensive Cardiac Care
- CCCIP-01 High-Intensity Statin Prescribed at Discharge
- CCCIP-02 Mineralocorticoid Receptor Antagonist Prescribed at Discharge
- CCCIP-03 Cardiac Rehabilitation Referral from an Inpatient Setting
- CCCIP-04 Cardiac Rehabilitation Referral for Heart Failure Patients with Reduced Ejection Fraction from Inpatient Setting
- CCCIP-05 Cardiac Rehabilitation Enrollment - Inpatient
Comprehensive Cardiac Care Outpatient
- CCCOP-01 Cardiac Rehabilitation Referral from an Outpatient Setting
- CCCOP-02 Cardiac Rehabilitation Referral for Heart Failure patients with Reduced Ejection Fraction from an Outpatient Setting
- CCCOP-03 Cardiac Rehabilitation Enrollment - Outpatient
The following are Advanced Certification Heart Failure chart abstracted measures used by ÃÛÌÒÖ±²¥.
Advanced Certification Heart Failure (ACHF)
- ACHF-02 Post-Discharge Appointment for Heart Failure Patients
- ACHF-03 Care Transition Record Transmitted
- ACHF-04 Discussion of Advance Directives/Advance Care Planning
- ACHF-05 Advance Directive Executed
- ACHF-06 Post-Discharge Evaluation for Heart Failure Patients
- AHAHF106 Defect-free Care for Quadruple Therapy Medication for Patients with HFrEF
- AHAHF94 SGLT-2 Inhibitor at Discharge for Patients with HFpEF/HFmrEF

ÃÛÌÒÖ±²¥ has chosen standardized chart abstracted cardiac care performance measures for five of the available cardiac certification options. The measures assess care provided to heart attack and heart failure patients, in addition to a cadre of measures that evaluate care provided within additional domains of cardiac care.
Following is list of measures for each advanced certification program:
